Policy Statement

The USF College of Medicine and the Program Director for each specialty and sub-specialty program are responsible for ensuring complete and competent training for each medical resident.  As a means of ensuring the competency of all PGY-1 residents for certain invasive procedures, a sample Procedure Log Card (see attached) has been developed for completion by each resident with signature by the supervising physician.

Procedure

Responsible Party                                        Action

Program Director Provides each PGY-1 resident with a Procedure Log Card at the beginning of his/her residency training.  (Programs are not required to use this Procedure Log Card, but may, as an option, use alternative methods for documenting this procedure experience.)

These Procedure Log cards may be obtained from the Office of Graduate Medical Education (974-9744).

PGY-1 Resident                                                 At the completion of each invasive procedure listed on the Procedure Log Card, obtains the initials of the supervising physician, continuing until card is completed.

Program Director Initials each Procedure Log Card once completed and files original in resident’s file maintained by the program.  Includes procedure performance information in the resident’s evaluation process.
