Program directors have a responsibility for assuring accurate exchange of information for residents who are transferring into or out of their residency training program. The purpose is to allow the determination of the appropriate level of education. Program directors are required to obtain written verification of prior training and performance for residents accepted in transfer and to provide such written documentation for residents who are transferring out of the program.
Program directors are required to complete an evaluation for all residents who are leaving their program, whether the resident has completed the program or not. For residents who are transferring to another program prior to completion of the current training, the written transfer documentation is sufficient.
For residents who are completing the program, the program director must provide a final written summative evaluation, that includes a review of the resident’s performance during the final period of education and should verify that the resident has demonstrated sufficient professional ability to practice competently and independently. This summative evaluation must be part of the resident’s permanent record, maintained by the institution.
Responsibilities:
For residents transferring out:
Program Directors: Sends a written evaluation (in addition to any verbal communication) to the receiving program director with sufficient information about the training received and the performance of the resident to allow determination of the appropriate level for training to continue.
GME OFFICE: Verifies that the program director has sent written information regarding the level of training and the performance of the resident.
For residents transferring in:
Program Director: Verifies in writing to the GME office that he has received a written summary of the level of training completed and the performance of the resident from the transferring program director.
Completes the transferring resident form and submits it to the GME office prior to entry of the resident
GME OFFICE: Verifies that the program director has received written information regarding the level of training and the performance of the resident.
For residents completing the program:
Program Director: Completes a summative evaluation of the resident’s total performance, including performance during the final evaluation period, and verifies that the resident has demonstrated sufficient professional ability to practice competently and independently. A copy of this summative evaluation will be sent to the GME office to be maintained in the resident’s permanent file.
GME OFFICE: Verifies that each finishing resident has received a summative evaluation, in writing, and includes this summative evaluation in the resident’s permanent record.
